| Duties: | Under physician's orders, and RN supervision, the LifePath Hospice and Palliative Care, Inc. (LHPC) Continuous Care LPN is responsible for the observation of patient/family needs and providing palliative and supportive care to the patient/family unit. As outlined in the plan of care, the LPN utilizes nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family until. The LPN will report to the appropriate LHPC RN any change in condition of the patient or family unit. The LPN provides emotional support during times of crisis to the patient and family unit.
Shift available is: 8a-8p (4 shifts are required per month)
Our coverage area is ALL of Hillsborough County. The selected candidate will be required to drive anywhere within Hillsborough County to care for their patient. Reliable transportation, automobile insurance (minimum of 10/20/10) and good driving record are required for employment. |
| Qualifications: | Graduate of a school of vocational nursing.
Current licensure in the state of Florida
One year of experience required; preferably in an in-patient facility or med/surg unit.
Hospice/Home Health experience preferred
